Model Release Form

Hi, can anyone help me with a Model Release Form.

I have been asked to take some photos for my friends, daughters prom.
There will be about 7-10 other girls on the day, at the house and then they move on to school for their party. All the girls are 15-16 years old. Im just starting to build a portfolio up and would maybe use some of these photographs for my file. Any advise or a template form would be appreciated.

You should be able to find some good templates on the net, however as I don't use them I don't know where they are.
