In all likeliness you have configured your game(s) with a resolution and/or refresh rate your monitor does not support.

What games does this occur with, all or just some?

@Steveocee: I forgot to mention i use HDMI cable.

@sin chase: As far as i have seen it happens with most games but it is completely random, sometimes i can play for 3 hours with no problem so i cant see a pattern to it. Also i can play the game perfectly fine for the time up to when i lose signal. When i do lose signal i have to push the reset button, its the only way i can get any output being displayed from the computer.

I had this issue, and its my PSU. Mine is also a Modular and if I use the two PCI power cables that are bulit into the PSU I get black screen.
My fix was to use another two cables from the box.

I see your PSU is same setup has mine, you see that 24pin and 8pin and two PCI power are stuck inside the PSU?
Take another two out of your box and use them on your GPU and test.

Post back..

I really wouldnt know what i am doing with cables inside the pc i installed everything else but the cables i needed help with, so what is it that i have to do because i already am using 2 cables form my PSu because the wire wasnt long enough to get to my SSD.

You have two PCI power cables going from the PSU into the GPU now yes?
Power of the PC, and unplug the two cables that are into the GPU. Now go and look inside the PSU box and you should have a couple more cables, look for the PCI cables you should have two. On the PSU plug the cables into where its marked PCI and then plug them into the GPU.

Now boot up the PC and test some games, if you dont get any black screen, just hide them other cables at the back of case if you have the space for cable management.

This nothing to do with the cable, honest its a power issue, not enough to power the GPU and its just cutting out.
Are you sure?
Couldn't it be related to the various "Driver not responding" issues that regularly crop up in this (and other) forums? The one that causes the screen to go black for up to 5 minutes before the driver "recovers" or BSODs?
Look in the Windows Application Event Log (Windows Logs | System) for entries with a Level of "Warning", a Source of "Display" and an Event ID of 4101.
On the other hand, I suppose a GPU power issue might also generate a "Warning" 4101 event in the log. Either way, it's still worth checking.

Here's someone with the same (or very similar) problem: http://forums.overclockers.co.uk/showthread.php?t=18425274

Happens with my 680 quite a lot too and the red screen of hell when exiting games. The red screen of hell I have to turn the monitor off for about 3 mins or so, once turned back on after that time frame all seems to be right again. I do get an event issue spring up and it's a video driver problem. The black screens tho totally kill the pc as in TeamSpeak dies can't ctrl alt del, have to do a full hold in power button on front of pc for a forced reboot.

So this issue isn't limited to AMD cards as I get it too.

Gremlins I tells thee!

When I had the driver not responding that was down to a bad OC I increased the voltage and that fixed it.
OP didn't state he was getting that error so I was thinking back to when I had this black screen, I created a thread on here about couple months back and it was my PSU.

Thank you shankly, i downloaded the 12.7 beta now, ill play for a bit and keep you updated if it ever loses signal again. it may take a while as it has only happened 6 times since i first got the pc about a month ago.
