The cashback is a promotion from the companies involved (Intel or Gigabyte for instance) and not Overclockers.

Does the cashback form work correctly? Its only allowing selection of purchase dates between 14th june and 7th july, and asus mb/cpu combo selections for me.

I was going to have a shot at cashback from gigabyte+cpu combo bought on july7th, but the options don't allow it (just asus).

I see the list of qualifying bundles, but I see no option to enter the corresponding bundle number anywhere at the start of the form.

Yes it works correctly. If you read the T&C's it states you can claim 30 Days after Promotion start Date but no later than 60Days After
