My original answer was entirely serious. You need to date it, you need to signal your intention to leave and you need to include your leaving date. Thats all you should be including.

Here's an example: Mine actually :)

XX XXXXX 2006.

Dear XXXXXX,

Please accept this as formal notice of my resignation from the position of XXXXX XXXXXXX, with effect from XX/XX/XXXX.

In accordance with my contract of employment I am happy to continue to work until XX/XX/XXXX, this is to allow time to arrange a suitable replacement and allow for a smooth transition of work responsibilities.

It is with regret that I am choosing to leave, but my circumstances have changed since I started my employment and I feel I can no longer provide the commitment required to be an effective team member.

I thank you for your support during my time with the company, which I have found enjoyable and fulfilling.

Please let me know the arrangements for handing back equipment, company car, etc and handing over outstanding work and responsibilities.

Yours sincerely

XXXXXX XXXXXXX.

Dear Boss

you suck so I'm leaving, you have 7 days notice as of "date" this will make my last day "date". may you rot in hell

Signed

YOU

I would suggest that the phrasing depends on the situation. If the departure is sufficiently acrimonious that a "pointed" letter won't do any damage that hasn't already been done, then however peurile it might be, by all means indulge.

If, however, the departure is friendly, I'd suggest something that :-

1) contains the necessary facts, like last date of work
2) gives a positive reason for leaving.

In my view, 2) can be important. If you thank the employer for the experience you've gained, but tell them that you need to develop your career and an opportunity has cropped up that is too good to miss, you won't (hopefully) put their nose out of joint. That means two things. Firstly, the opportunity to return in later years should still be open, and two, they won't still be miffed when they write your reference.

Bear in mind that when an employer writes a bland reference that merely confirms dates, they are doing nothing that can be criticised, yet the recipient may well wonder about what may be hidden between the lines. A glowing testimonial is far better.
