The number is withheld by the terminating exchange. So they do have the number, but with-hold it, if it says the caller withheld their number.
If the number is not available, then there is NO CLI with that call. Not that there is but its been with-held, simply that there is NO CLI. It may be a call from abroad, initialised at an exhange which does not support CLI, etc.

I know from my own experience many international numbers do not. So for all intents and purposes the number is withheld from you.
