My instructor has been throwing as many of these things at me as he can and from what I can gather it's as others have said. You needed to assert yourself more at the junction. You're turning right, act like you're turning right. Sitting behind the line with your right indicator on isn't good enough, you are blocking the flow of traffic and aren't going to be ready to go right should a safe gap present itself. If you don't move in to the junction you could literally sit there all day holding up traffic.
I can't see this being a major all by itself though. Likely you got a major because the examiner didn't feel you'd driven well enough overall. Maybe you hesitated a few times and he didn't feel that you were confident enough to drive solo? That's what it's all about at the end of the day. If the examiner feels you're good enough to drive solo you pass. The majority of faults can be written down as minors if the examiner feels it was a one-off and didnt present any danger to anyone (e.g. touching the curb when parking). If the examiner doesn't think you are confident and able to drive alone then you tend to fail and usually for things that will seem silly to you. It's not necessarily that one thing (the major) that made you fail, but rather the whole drive. Some faults can be major or minor depending on how the examiner perceives your overall performance.

Id have considered it a minor but all the same, the light turned green so you need to advance past the line and either take a gap in the turning traffic or wait for someone to let you in.
Its a little worse that you were coming to the line and saw it turn green, there was no danger it would change back as you were passing the line
All in all, not the smartest thing to fudge on the test and fail if you would have otherwise passed. Chalk it up to experience and move on

Don't worry about it too much. The way I look at it, after passing my test, if I continued to drive the way I was taught to, I would probably be either dead or severely disbabled by now.
