Problem exiting Quake 4

When I finish playing Quake 4 and exit to windows I don't get my screen (desktop or start bar) back. I just get a completely blank screen with a mouse icon.

I can execute keyboard commands but more often that not I have to reset the machine. Does anybody know what is causing this?

I don't have a return to desktop button...

I have the latest one installed - and have performed a roll back, but nothing still really happens. I have also installed an update to v1.3....

I have 2GB DDR2 memory - the trouble is nothing appears!! Not even CTRL ALT DEL!
