What issue tracking software, for use in general business, do people recommend?

What sort of issues

Examples would be software issues and document corrections. I'm interested in workflow improvement systems where issues can be assigned Ids and put into a queue for a specific person to attend to. Really looking for a flexible issue tracking solution that can be used to improve general business efficiency. Thanks.
