I think when you need health it should take you to a sub game.
In the sub game you would be shaking, depending on how hurt you were, while you try to guide a needle into an open vein, if you get it wrong you accidently give yourself gangreen buy peircing an artery and your arm falls off over time, if you run out of ammo you can use this as a last resort, ala stubbs the zombie.
