Wouldn't happen. You'd've changed lanes before you got to that exit/entrance. Plus as jamoor said above, you'd be indicating. I think the main misunderstanding with the roundabout is that the dual carriageway is straight on - there is no left turning. The second exit is essentially right, and the third exit is almost a complete 360 - as I said.
