RoundCube Webmail - Help

I have been trying to install RoundCube on my hosting and I have got as far as the login page which I can access by going to http://mydomain.com/roundcube

This brings up 3 boxes: username, password and server.

I have no idea what I need to enter in these boxes to login

The only username and password I have setup so far is for the database.

Any ideas?

Thanks for that worked fine and also solved another problem I was having.

I couldn't send emails because I was entering mydomin.com instead of mail.mydomain in the server box
