Definitely doesn't happen with Dreamhost. I've seen more than a handful of people who've used TBs of bandwidth a month and Dreamhost have been fine with it.
It's hard to satisfy that amount of bandwidth on a dynamic website without ruining it for the other people on your server, though, so they occasionally stop people running CPU-intensive scripts lots, but that's more than reasonable and as a regular user I'm glad they do it—and let's face it, if you have a site that's getting that many hits, you should be able to afford a dedicated server.
