Sharepoint, Word and a database - how?

Hi folks,
OK, there are three objectives to this thread.
  1. Find out if my idea if possible.
  2. Find out any tech jargon needed to frame the concept more effectively.
  3. Actually work out how to do it all. :D

OK, here's the idea. I want to allow people to create Word comments from a template stored in a Sharepoint site. They then print that form off for wet-signature (as archaic issue in itself). However, when they print it I want to extract the info in the background into a database, spreadsheet or similar (ideally Microsoft based). This then allows me to do some data analysis later. What happens to the hardcopy document is already managed in another way and outside of the scope of this project.

I need all this to be simple and with minimal hassle on the part of the user.

So, thoughts, please?
