The fan controller you have posted fits in a floppy disc bay, however you can also get the optical drive bay ones too.

The temprature wires coming out of the fan controller are temprature probes. You have to place these either on the heatsink or the core of the chip you are monitoring.
Personally ive just used one to monitor the case temp which gives me a indication of the heat inside the PC.
