Spec me an Eyefinity setup?

Not even mine? The only crossfire setup suggested.

With Crossfire you can only use the outputs from one card.

You setup would probably work connecting one monitor using a DVI to HDMI adapter, the second monitor HDMI to HDMI and the third monitor with an active miniDP to HDMI adapter or possibly active miniDP to DVI and then a DVI to HDMI adapter.

2 7850's is more than enough grunt for 5760 x 1080
connect one monitor to the his display adapter and connect the adapter to the 1st gpu.
connect the other 2 to the cards using dvi.

the case you can change to whatever you like, it is a personal preference, the whhite 810 swich is what i have and love it, but it is £150

the monitors are the most importatn part of an eyefintiy setup. i went for the samsung syncmasters as they have the thinnest bezzels arround and that is part you should be looking for when buying 3 monitors. i went for the 22" syncmaster and they are excellent.
