Well there are two tablet options. An iPad, or if your are poor a Google Nexus 7.
Swap the nurse outfit for a woman in a dominatrix outfit, a cat 'o nine-tails and replace the cream for salt (to rub in the whip wounds) and I think you have my perfect birthday list.
