I've just started looking at code academy, looks good. I use expression which is ok tbh. Especially at that price.

When designing nice modern websites do people use the grid system?
What do mean? People use grid systems as an aid for good design.
And people use grid systems in premade css scripts to speed up development.
Different applications...
No you don't have to use the premade css scripts... You can do it completely from scratch.
If you want to turn a design into website, there are plenty of online tutorials like this one
