The problem seems to be within hour's play, suddenly the game will CTD or completely hang my system to the point where i have to do a hard reset. All other games are running fine without issue, C3, Fallout 4, Rise of the Tomb Raider.

I have recently upgraded my video card to the one seen in my sig, running 364.51 drivers also.

Things iv'e tried so far are:

Reinstalling Nvidia drivers (clean install)
Have kept an eye load temps, they are fine.
Verifying game files in Uplay
Ran other games for at least a couple of hours, all checks out fine.

I'm leaning towards the game being an issue here, but could use some more light shedding on this, just in case I'm missing something here? Cheers

Remove your overclocks and your find the game will be stable.

It happened to me, where for about 9 months on Win 8.1 my OC's where 100% stable. As soon as I upgraded to Win 10, I was getting Watch Dog violations and blue screens etc.... in games which where 100% stable for months previously.

No amount of tinkering helped, so I removed my OC and now I am 100% rock stable.

Try it at least to exempt it as the issue.

Different software will stress your system in different ways.
