Triple Monitor 'device disconnect'

I have a triple monitor setup with 2x LG L1720B and a LG IPS231. These are connected to a single 6850 with one of the L1720B's connected using a "Sapphire active displayport to vga adapter".

The problem I get is when I run an application that is not still (e.g. a game (even a flash game like tetris online) or skype) the monitors will flash black for a second and I hear the windows 7 'device disconnect' sound. After which all my windows are rearranged. Depending on the program this will either keep happening until the program is closed or will happen every couple of minutes or so.

If I play a PC game like crysis 2, once the game starts I get constant flashing and the game will eventually crash.

I have no problems if I disable all but the main monitor (ips231).

Any ideas?

yes i'm on the latest drivers, also Iv'e updated the drivers a few times and the problem remains.

My card is overclocked, not sure of the PSU but its a 750watt, had it for ages, its fine. I have not resitted the gpu for a while but I don't think thats the problem, one one screen I can game for hours.
 
Have you tried going backwards on the drivers? Sometimes a driver can add a problem or compatibility issue.

Is the active adapter the type with an additional usb plug? Try moving it to a completely different socket (if there are2 stacked, suually they are on the same channel).

Try swopping monitor cables about? Got spare cables? Try one, swop, try a different again etc.
