Probably a really obvious answer as to why this hasn't been done yet, but to solve the fps problems people get when enabling v-sync, would it not be better to have monitors that have variable refresh rates?
In other words, instead of a fixed refresh rate (e.g. 60hz, 120hz), the refresh rate would be determined by the fps you are currently getting. Obviously you'd still need a cap on the maximum refresh rate (hence a cap on fps), but at least you won't get tearing problems or horrible drops in fps, since the screen will refresh as soon as a frame is rendered (unlike how it is currently, where the frame has to be held back untill the next refresh).
