Nah it isn't that, happens with Vsync on too. Running down a road in Skyrim, all smooth, no tearing as the camera isn't moving but every few seconds the character skips ahead a few steps, bloody infuriating.

Could it be the iFPSClamp option in the ini file. Most of the games you listed has a well known skipping issue and you need to change the iFPSClamp to your refresh rate to stop it.

"C:\Users\[your username]\Documents\My Games\Fallout3\FALLOUT.ini" and change iFPSClamp=0 to iFPSClamp=60 ...
