Might be a Cat5, dunno. 6 poles, looks like a standard phone plug. It's used for connecting a PIR to a lighting control module.
 
RJ11 jacks are 6 pole, RJ45 (usually used with CAT5) is 8 pole.

Are you interested in the connector or the cable?
 
Doesn't matter what it is really. Any low voltage signal like that could be sent over Cat5/6 just as well.

Rip it out and replace :)
