What's the problem with this PDF?

When you "tick" the boxes and then scroll down they drop off: http://www.newport.gov.uk/stellent/groups/public/documents/form/N_006206.pdf

Any idea what is going wrong and/or how to solve the problem?

Does it work if you tick "Internet" (Post Details) and then "Work Permit = no" (Personal Details) then scroll back up to Internet?

Thanks dude. Fortunately I'm using Pro and was able to inspect/rename the fields as you suggested - worked a treat!

I think the IT people at Newport Council need to spend some time on OcUK
