What's the problem with this PDF?

I can't even open that PDF using Acrobat Reader 7.0.7, it tells me that "Acrobat Read could not open 'N_006739.pdf' because it is either not a supported file type or because the file has been damaged (for example, it was sent as an email attachment and wasn't correctly decoded)."

It works now. I can tick the boxes, scroll about and the ticks stay in place.

Does it work if you tick "Internet" (Post Details) and then "Work Permit = no" (Personal Details) then scroll back up to Internet?
 
You need to give the groups (of check boxes) different IDs - eg WorkPermitYes/WorkPermitNo, SawPress/SawInternet/SawJobCentre/SawWordofMouth.

Because they're all called "tick", they're all grouped together as one (so you can only select one out of the group).

Doubt that's obvious in Acrobat Reader. Shows up in Acrobat Pro when you select the Check Box tool. :)

Thanks dude. Fortunately I'm using Pro and was able to inspect/rename the fields as you suggested - worked a treat!

I think the IT people at Newport Council need to spend some time on OcUK :D
