Where do you put sound proofing?

Hi,

I am having to add sound proofing to my drivers door due to it not having any at the moment. Now where abouts do you put it, I know you put a layer on the inside skin, but then do you put some on the outside skin directly behind the speaker?
 
Glue it to the door skin to stop it drumming and any panel like the floorpan, roof, boot etc. This stops huge amounts of noise being transmitted through the panel itself.

What you using, some of that rubberised sheet like Dynamat?
