Gentoo can be installed in a number of different manners, the 50mb will be a basic command line OS which you install onto a hard drive, it will download the most up to date packages unlike the other bundled CDs, you can also select your kernel to download.

The other CDs are bigger because they have some packages already in them from the repository which is really used for an offline install, those packages wont be as completely up to date as the minimal cd since you will be downloading the latest.

The Live CD is basically an OS with a desktop system (gnome) which loads up, from there you can use the GUI installer which is really just a bunch of point and click.

Note that no matter what CD you download (ie if you download the live CD) you can still do a command line install and merge the latest packages if you wish.
