I just read up on that story ... "Firefighters said the grid cover was broken some time ago and a piece of cardboard placed over the hole as a temporary measure."

Cardboard?! Who's bright idea was that, people could easily mistake it for litter and not know there is a drain underneath, an accident waiting to happen IMO. At least use a piece of wood or something.
