People are still dying in prisons similarly there afaik. Preventable blindness and cancer treatment/scanning, lots of neglect seems to be part of the meta.
It's a work of art but I wouldn't like to use it for real. The blade is fine but the hilt looks to be a waste of effort. The ivory is going to slip in the hand, and it's too big for one hand and too small for two hands. That pommel isn't going to keep a hand from slipping, nor is it going to help thrusts.

A cardiologist who's also a comedian, with a Youtube channel. The context was a standup routine based on unnecessary detail about cardiology. How's that for niche? I think he did a fine job of it.
