Hardest I've got are prob doing all of veteran on cod and mile high club, the one free bullet on Hl2 episode 1 isn't that hard tbh and it's pretty fun.
I registered with that true achievements site to see how I fared - my best achievements are 100% GTA4, Gears2 on insane and Bully 100% - which tells me not that many people bother to get completionist achievements on games.
Was an interesting method of giving games scores though there are obviously flaws in its points system - the main one being that if youre the only person to have the achievement its value rockets - that doesnt necessarily make it a hard achievement - it might just be a long grind or something..
Hardest one for me so far (which I still haven't got, but determined to do it) is the single bullet on The Orange Box. Get so immersed into the game I just think "ahhh sod it, eat lead muffa" and then realise what I've done!
With the amount of lag you tend to get in that game and the fact you may:
1)Not get any experience points if the game selects you as host and your connection drops for just a split second.
2)Lose 1500 exp if you get disconnected for any reason AND not get any exp points the next time you play.
I'd say it might be hard to get.Haha!

One of the hardest I have seen and one that I actually have achieved is the Bladder of Steel award in Rock Band 2
For those that don't play the the game to get the BOS you have to play all 84 songs in the game back to back without failing a single song or pausing the game once.
