The IPCC didn't actually say that though. But its a reasonable guess.

OK thanks, it's clearer now. I didn't know two cops were involved.

So... one cop in front of Duggan; the other behind him. Cop shoots Duggan twice; one shot passes through and hits the other cop. Makes sense.
