You're not playing in Windowed fullscreen are you? I was having the exact same problem and then switched to fullscreen and all was wel.

I've tried both Windowed and Full screen still exactly the same mate.
Different settings seems to solve the issue for different people, it seems very temperamental.
As said it can go through periods of being perfect and then back to being a stuttered mess.
Try making a post on blizzard forums under customer under technical support?
People always complain that its full of trolls but ive had mostly a good experience with help on there.

This problem has nothing to do with AMD cards. I'm using nVidia and I have it. It's purely to do with server lag.

Run a "netstat -b" look for the D3 executable in the list, it should give you an IP address that the game is connected to.
Run a "tracert" against that IP, you can also try WinMTR for some longer insight into if there is a network related issue.
I've not had any stuttering or lag issues at all since launch (other than the login issues at the very beginning), I'm running it on an Q6600, 8GB, HD6950 (12.4 drivers I think), Sky 14Mb internet & game is installed onto my RevoDrive.

And the fact if you don't farm you won't reach 60 until Hell. So more skills and runes as well.

You have randomised events as well.
Means it can be raised in a future expansion too I would think.
Hopefully the game won't get too stale replaying the same acts over and over. I hope atleast anyway.

I'm with TalkTalk as well and they do traffic shaping during peak hours. They remove the shaping at midnight, so it might be that.
Just finished last boss in act 1 on nightmare.
When I did him on normal I stood them, just held m1 and he went down in seconds.
This time round I actually had to move around, use health wells around and it took like 2min to kill him.
So far nightmare makes elits pain in the bottom, normal packs are still easy peasy and bosses become more fun as they require more strategic approach.
