the invisible soldiers on FEAR scared the crap out of me, its like wtf where are they gonna come from next?! i think they could climb walls too. it was a seriously panicky moment for me

another one is demon gate from ff7, it actually bought tears to my eyes at one point i was so infuriated. (its easy now i have a routine on how to defeat it)

I went a bit shotgun crazy in that part of the game, blasting everything.

Yeah me too, I was fairly new to FPSs back then (having pretty much only played Doom) and hearing the Shambler roar meant unloading all your ammo into him and trying to avoid anything he sent your way.
I went back and played Quake again recently and found that circle strafing and all those other little tricks you learn makes him quite easy to kill. The Vore is stil a pain though.

Definatley up there. The sheer panic caused by one of those bad boys coming round a corner hard to equal. Let alone trying to deal with one in close terrain without losing half your men.
I remember levelling entire buildings trying to flush those out.

The Fast Zombie's from HL2 scared the living hell outta me when i first played through and one of them came charging at me,

The Witches in L4D can pretty scary if one of your teammates triggers her and the first time a tank comes charging at you as well,

And finally Dr Robotinic from sonic just for a being an absolute **** was always laughing at me!!
