Church and barn wedding.

At least for wedding work, atmosphere, content and emotion are what I care about most. I could add a flash to reduce noise, but it would also change the atmosphere from how it really is. I may need to wait for my flash to recycle, and miss content (happens occasionally during dancing) and so on.
It's a compromise (as always :p), but I value allot of other factors above noise. In black & white, I'd be happy with a 200" ISO 12800 print. Yes the noise would be there but I like it, like I like the sound of crackling Vinyl over clean MP3 files sometimes.

I totally agree, if it was up to me all my clients have an awesome grainy black and white only album! I remember when I first started I'd try to expose everything perfectly as if it was in the studio, now I just go with it. Really dark room and loads of window light, silhouette it is! Pretty jealous of the shadow recovery from the latest Nikons!

I generally don't use flash at all until the first dance, and even then I never seem to be happy with it.

what vsco presets do you use on your images?

I don't use VSCO presets. I have tried them out of curiosity and some are kind of similar. According to Raymond & I agree as well, Kodak E100G is close to my processing. (use some fade to lift shadows etc though) unless you prefer a little more contrast.
 
I generally don't use flash at all until the first dance, and even then I never seem to be happy with it.

I sometimes use bounced on camera flash if I don't feel brave enough.
I don't use it, but I do think backlit flash can work really well. For times I need a little something to bolster the ambient, I'm tempted to use a flash on camera purely for AF assist, and as a commander to fire off a subtle backlight (with warm filter on).
The only problem is I move around allot so some of the pictures will have a nice backlight, and some would be bare direct flash. I would have to find a way to enable/disable them quickly on the fly.

Yeah I like the backlit look, and I know what you mean about moving around. I've just bought a full set of pocket wizards with the camera mounted controller. Not had chance to use it at a wedding yet but there are 3 groups on the controller and you can instantly flick to auto, manual or off for each individual group.

In theory it sounds like its going to be great.

You need at least 3 off camera, one on camera as fill.

Place 3 flashes in the room so its a triangle to cover as much ground as possible.
