Fixed at the front. One pulsing and one fixed at the back. I was cycling home on a canal towpath last night in the dark and some tool had a ludicrously bright front light coming towards me set to pulse (pointing at my face rather than the path, because of course it was). It was really difficult to judge their position and speed.

I'm not a cyclist but as a driver I always think it's easier to pick up a bike if the have both a constant and flashing/pulsing light front and back and people with really bright front lights aimed high and mega bright helmet lights that they stare at you with drive me mad. Those that choose to commute through our local park like it is a championship racing circuit annoy me the most though if they are in such a hurry there is a perfectly good road round the park!
