How do windows upgrades work after format?

Let's say I buy a Windows 8 upgrade. I assume the first time, I bung it on top of my existing Win 7 install. But what about a fresh install? Do you install Tue original OS and then apply the upgrade, or just install directly to the newer OS?

I realise no one knows tbs for sure for Win 8 yet, so how has this worked historically?
 
Years ago when doing a fresh/clean install you'd be asked to insert the CD-ROM of the version of Windows that entitled you to use the upgrade version. For example When installing Windows 98 you'd be asked to insert the Windows 95 CD and once verified you would reinsert the Windows 98 disk and carry on with the installation process.

I have no idea how Windows 8 is going to work however and it's a good question.
 
Right now it's looking like you'd have to install the full OS you have the license for then pop the upgrade disk in. Not a huge fan of doing that, I always prefer totally clean installs so I'm crossing my fingers they just ask for a valid key.

You can do a clean install using a upgrade version if you like, or bung on win 7, then the upgrade.

To avoid all of this faff, they should just release the 'home' version for £50 and the pro version for £100. And allow users to upgrade using these versions
