I have never had a card fail (lucky I guess) and even when trying to kill an 8800GT by running Furmark on 320.18 drivers, it still won't bloody die
I still look for the best RMA and lack of downtime/customer service.
I will get jumped on for this but I do notice a lot of the AMD cards need returning but that could just be coincidence.

Interestingly, I remember a thread a long time ago which was about failure rate of graphics cards among forum members, and I think up to but not including the GTX400 series, Nvidia seemed to have significantly worse numbers in that regard.

I've never had a card die on me one did arrive DOA, or rather it blew out my PSU and damaged the motherboard. Was quite spectacular, a bang, a blue flash and a puff of smoke. Must have had a short circuit somewhere.
Got a replacement under RMA but never got a refund for the replacement PSU. Even had to pay return postage.
This wasn't OCuk and it was a good many years ago, maybe RMA procedures have changed since then.
