You may need to buy a new cooler for the graphics card then if it is still making them noises, but is that graphics card any good ? I can't even tell which one it is there. Can you list its model number and make please. You maybe better off buying a new graphics card then investing more money on an old card, but lets see which card you have first. Fans die in different ways and it sounds to me that the fan on that card has died or the circuit that controls its speed is on the way out.
NVM: I just read up its a Nvidia 250, maybe try find a cheap GPU cooler for it mate.

Also I'm sure is the GPU, I listened to it when I opened the case.
It could also be a damaged GPU and overheating and not the cooler and fan causing the problem. You have to decide if it is worth testing a new cooler on it, but have you made sure 100% that the cooler is on the card correctly and tried new thermal paste too ?
