Ignore what the guy says in the description, this is working on Vista 64 as I write this. Also, if your system is already patched to use custom themes, you won't need to do any patching or use programs like Vista Glazz.

Put the theme files in your Windows>Resources>Themes directory.

The theme should then be selectable from the drop-down menu in Personalise>Theme. Sometimes the theme will be called Aero (the same as default).
