I did some extra stuff in Photoshop after as well, but this should help.

You might have to make some tweaks to suit the lighting of your shot, for example I used similar settings for this but I had to change it to suit:

Where would place that file then sir im just being thick today
 
You can put it anywhere, you just have to right click in the 'Presets' pane of the 'Develop' module and then click on 'import', select the template and go for it.

Do remember though that you will likely have to tweak it. I have loads of personal presets I have created, and some simply do not work on anything but the shoot I used it on, because of the lighting.

PS. I can't believe this thread is still going, so much has changed since I started it.....in a good way of course!
