Brought down a reasonably large (but poorly configured) network by plugging a cable into an unlabeled socket. Could have happened to anyone but I still felt a right numpty.
Working on an old pc at work and I accidently snapped one of the cpu heatsink retaining lugs on the motherboard. Ended up having to superglue the heatsink to the processor, worked for ages after that
