I have 2 x SP120's and 4 x 200 mm fans connected to my fan controller and I hear the dvd spinning up over the fans ... do you have a H100 or H100i ?
If you have the H100 check the motherboard has the correct heat curve set so its not just spinning the fans full speed if you have the h100i make sure you have the software installed and the correct heat options sec also have you got the fans on push pull ?? as I have found that helps with the temps.
That's pretty hot, I recommend you download EVGA Precision, open up the settings and select the 'Fan' tab, at the top of that there should be 'Enable software automatic fan control', you should check this box and click 'Ok', now, load up a game and see what it hits.
Another thing you can do is in this same software, open up the 'monitoring' tab, and select 'GPU Temperature' and check the box at the bottom of the window 'Show in On-Screen Display' and hit 'Ok', now go along to the 'OSD' window and select a key to setup 'Toggle On-Screen Display'. Now when you load up a game, you'll get the Temperature reading in-game.

Ccc should be handling the fan speed at the moment. Which isn't any different from other 3rd party programs. 91 degrees after 5 mins and full speed fans is a faulty card.
There is a forum section intended for just this sort of eventuality.
