Which Total War?

I've played through Empire several times, I love the setting, Napoleon is better but lacks the world scale of Empire.

Adored Rome, played it so much with the RTR mod. Can't stand Rome 2
Never played medieval 2, no idea why not.

Well, you're probably best ignoring this, but for me it is still Shogun: Total War (yep, the first one). You can just put it on and get straight into it imho and I really like the capturing of entire provinces over the more tile based system later on.

Empire or Napoleon with Darthmod, some of the battles can be stunning.

Watching some of DiplexHeated early stuff on youtube really helped me when learning the game also

Empire Total War: Napoleon with Darth mod was very good , it was a welcome break to swords and shields from previous games.

Shogun 2 Fall of the Samurai delivered what I've been waiting for in a TW game for years and years, a fast firing chain guns which you can take control of and mow down lines of infantry like grass. Very satisfying.

Still to try Empire and Napoleon here, but I enjoyed the campaign most on Shogun 2, Rome 2 and at times on Attila. I'm just getting into the Warhammer game though.

Medical 2 total war with expansion and stainless steel mod, spent tonnes of hours on that. Also has an amazing lord of the rings mod called third age.

Shogun 2 total war and fall of the samurai with darthmod.

Rome 2 is really good with mods.

War hammer total war really good fun, should check it out after you put about a 1000 hours into the others

When Medical 1 came out, and there is medical 2?
Are viruses and bacteria the factions, fighting each other? Are there any doctors with antibiotics, or anti virus jabs?

Is the NHS a fighting factions?

(I know is wrong spelling, but had to make a joke because found it funny :D. hahahah
I do not mean any offense mate, and accept my apologies if perceived did so.)
