I wonder if the assumptions are based on Western methodology which I assume would be along the lines of six sigma.
3.4 occurrences per million, ie 3.4 barrel failures per million at the intended replacement point. We value our troops lives highly and would have a bar set at a very high level (such as that)
I would hazard a guess Russia would apply a somewhat different metric. (use them till they blow, people are cheap and plentiful)
