They must be able to build replica guns by now that can simulate the noise and smoke well enough for films without even having to load blanks, how hard can it be!
Very easy, there's no need to use live guns in filming, none at all.
CGI and sound effects have come a long way.
That said I don't think there was any intent to harm here, it was an accident, wreckless, stupid, irresponsible, and easily preventable.
Hopefully this will serve as a lesson to the film industry and will see an end to firearms being used as props.

Loads of film productions use firearms absolutely safely.

The reason is to make it look real. The CGI looks crap in comparison. Cars kill more people in film production than firearms do, so why bother using those as well...For no apparent reason.
Talking in terms of risk management, if the actor can't hold a fake gun and fake the recoil, then they are a bad actor.
Likewise if the effects team can't do a barrel flash, sound effects, and smoke, they are a bad effects team.
There's simply no reason to use real guns when making a film in this day and age.
you just need to follow the rules.
