FWIW, Kotaku's Jason Schreier has said this;

Some fans have also theorized that this might be some sort of remaster of Fallout 3 or Switch port of Fallout 4, but from what we’ve heard, this is a new game, one that takes the series in a brand new direction. We’re expecting to see more tomorrow.

It won’t be online, as that would take current and potential players away from ESO.

It’ll be an expansion like new Vegas was to fallout 3.

That’s my bet.

I've read one theory saying it'll be a FO4 expansion that sets up the Starfield universe (which is rumoured to be technically the same universe, just further in the future). I wouldn't object to something along those lines.

The E3 presentation will be at 02:30 UK time so it may be a few hours yet, if in the next 24 hours at all.
It's a massive audience for the equivalent of watching paint dry.
