You’d think this stuff was advised to the players before the game. A keeper is in a win win situation for pens, I’d be gutted if I couldn’t take part in a penalty shootout.
Cabalero is a good at pens...for all we know this could have been planned and the "injury" coincidental.
 
According to Transfermarkt, Kepa had conceded 17 of 21 penalty kicks faced.
Caballero 17 of 28 faced.
(Admittedly only includes pens faced during matches)

I think that is exactly what happened, my take is Kepa was wasting time by faking cramps. He thought Sarri thought he'd have trouble during penalties and was a big risk so wanted to swap him. Sarri knew it was time wasting but wanted a keeper he saw as better for penalties on the pitch, also a keeper who trained penalties with the likes of Aguero, KDB, Silva, etc for years.

Still stupid and he has to come off, but I think that is the reasoning. Kepa thinking he was only being taken off due to risk of cramp when he was sure there was none as he was faking.

Substitution procedure

The names of the substitutes must be given to the referee before the start of the match. Any substitute not named by this time may not take part in the match.

To replace a player with a substitute, the following must be observed:
  • the referee must be informed before any substitution is made
  • the player being substituted receives the referee’s permission to leave the field of play, unless already off the field
  • the player being replaced is not obliged to leave on the halfway line and takes no further part in the match, except where return substitutions are permitted
  • if a player who is to be replaced refuses to leave, play continues

So no punishment and a player refuses to leave they don't have to apparently. Player will almost always leave the pitch even if angry afterwards because it's better to be angry than have the ref bench you for however many matches they choose due to your behaviour.

Probably, but if players aren't going to respect the rules then theoretically a player who gets a red card could decide to just plonk himself in the centre circle and not move, I'd imagine the referee will have the power to get him physically removed from the pitch by stewards.

As a former ref I can tell you that if a player is sent off and refuses to leave the pitch then the match is abandoned. Depending on the individual rules of the competition the current result may or may not stand, or the game would be replayed - that usually depends on how much time remains.

However I am not aware of any power the ref has to force a player who refuses to be subbed to leave the pitch, I suspect something will be added to the rule book for next season though!

In theory I guess as it is private property he could be forced to leave.

In the same way you might eject a fan who is disrupting play.
