I've noticed there isn't a topic for this, yet GTA IV has to be one of the more frustrating games to mod, and has such a gigantic selection of mods to choose from.
I don't really mod the game too much myself, as I have several issues, but I will compile any mods people post and talk about, as well as tweaks to improve performance here.
GUIDE TO MODDING


Tweaks

By far the best tweak for GTA IV has to be the commandline.txt, simply create a notepad .txt document in your GTA IV directory and add these commands to it to increase performance.

ADVANCED USERS ONLY;

A COLLECTION OF ALL THE Commandline.txt commands i have found and that have helped me


[AUDIO]
-Fullspecaudio - high-quality sounds, but the load aj ¹ ¹ ??¿ce more processor
-Minspecaudio - sounds low quality (no digital processing), cut the aj ¹ ¹ ??¿ce processor



[GLOBAL]
-Adapter - use the screen Specified adapter number (zero-based)
-Benchmark - the game immediately in the connector and the benchmark results of the connector you're
-Disableimposters - O connector low quality models of cars as seen from afar
-Help - displays a list of commandline options
-NoBlockOnLostFocus - do not block the window update When It Loses focus
-Safemode - sets the low end graphic quality Sheva
-Uninstallcontent - removes zawartoϾ GFWL in GTA


[GRAPHIC]
-DX9 / high - in Connection with the support playing DirectX 9 and a high priority
-DX10 / High - in Connection with the support playing DirectX 10 and high priority
-Forcehighqualitymirrors - the connector reflections better on 64 bit systems
-Forcer2vb - improves the activity ³ ANIE dents car body on a clean game without patch
-FrameLimit - limits the number of frames to ...
-Framelockinwindow - ¿odœwie YOUR game 'window' ³ activities and preferably at 60Hz
-Fullscreen - pe ³ ny screen
-Gpucount - lets you manually set the GPU count if the query fails
-Height - Altitude set the [minimum 480]
-Managed - use d3d runtime managed resources

-Noswapdelay - loads all facilities instantly

Be careful when using them however, as it is hardware dependant, you have to choose tweaks depending on screen resolution, refresh rate, GPU memory, system memory etc. and choosing poorly can result in the game being forced into 800x460 resolution with everything on lowest.

Icenhancer is by far the best post-process 'ENB' mod available for GTA IV, do not be intimidated by it however, as it actually gives BETTER performance than standard GTA IV, while dramatically improving the visuals.

I don't think its multiplayer but its very similar to DayZ, only instead of other surviving players there are Scavengers and Bandits controlled by A.I. They can attack you, or assist you by killing Zombies, the way the zombies work is awesome and so ingenious too, it just makes every pedestrian hostile, in the wounded state, trying to attack the player with their fists. As the game is standard, without any real modding, this looks incredibly a lot like real Zombies, and they don't run either!

I enjoy driving around the city, that's about it. The problem is Rockstar games don't officially support modding of GTA 4 so you have to find a way around the DRM's for the retail version. Rockstar social club i don't have issues with but games for windows live and securom are troublesome when you try to load or place any mods into the game folder, patched or unpatched. xLiveless 0.95b flagged a trojan inside it's folder although my av removed it straight away. SecuROM closes and the game refuses to load if i install any mod. It looks like only the steam version plays nice. I like the look of iCenhancer+realizm mods. Shame about the retail version, maybe have to find a way around the DRM's?. I'm hoping GTA 5 only uses Rockstar social club?.

Hopefully GTA V will be Steamworks with no restrictive DRM, the modding is pretty fussy, I've legit modded the image files for new cars etc multiple times successfully and then it'll just crash. Every ENB I've tried has sent my game crazy, screen tearing so bad I cant even see my minimap, but I got VizualIV to work, which is a pretty good ENB, less shiny than others but nice lighting. Other than that its all vanilla. :D
 
a couple of mods you might want to add to the OP. all should be available at http://www.gta4-mods.com/

- Realistic Driving and flying: makes the cars feel a bit nicer and the helicopters feel a lot nicer
- Viva New York: replaces some textures and signs and stuff to exactly what is in new york
- SparkIV: there might be better programs for this, but i use SparkIV to add new car models. i think it can add other stuff too


there's a couple more mods that i use but i forget what they're called
 
One of the best tweaks to get performance out of GTA4 is tricking the shadows into running at a lower resolution. Making the shadows/reflections think they're running at 1024x768 while the game is running at 1080p gives a huge performance boost for not too much visual loss. This page explains it a lot better.

The Realistic driving and flying mod was great, but I got absolutely sick of touching fire hydrants and the full car being flipped into the air.

The Icenhancer download link doesn't work, what do people recommend as the best and most balanced?

Thanks for that shadow tweak tried it but it seems to make the pop-ins even worse, makes the game run 10x more smoother though

ENB's are personal preference, I've only tried Ice and this Gionights

They both look very similar however to me Ice Enhancer looks better to me because of the lighting, the only annoyance I have is that during cutscenes they are way overly bright and although you can turn the ENB's off on the fly I get weird performance issues if I do that so for now I'm using none :p

I tried to install the latest patch after getting it on Steam the other day. After copying over and pasting about 4 diff dl's and hundreds of files it corrupt GTA 4 on me. I forgot to back it up and have yet to download it again.

Can someone that has it working from the version off Steam do a step-by-step guide on how you did it? The guides I came across weren't great. AFAIK the 1.0.4.0 version has the best visuals but no online, and 1.0.7.0 has worse visuals but is online compatible. I just want the best visuals for the story mode.

If someone does this it would be great! Thanks

I got the pack in the Steam sale but installed EFLC, it's annoying trying to get mods to work with it but it has the most mutliplayer stuff, which is what I do, basically mess about in multiplayer.

I have installed a few visual mods which broke the game, then repaired it using steam but some files were still left over so now I have 2 or 3 different visual mods that seem to be clashing... I need to download again from scratch =/ This time I will spend that extra bit of time backing up.

At the moment night + rain looks great but everything else is average and get red sky bug + extreme dark indoors and certain time/weather. What's the best way to enable AA?

Also might try that shadows tweak.
