When you want to update the firmware you have to connect the the controller via a good quality usb cable. I say good quality because I tried two other USB cables and the pad wasn't detected by the app.
I used my ps4 cable.
My procedure was as follows,
1) left pad turned off
2) connected via USB (if the cable is good you will hear the windows USB alert when you connect it)
3) started App
4) the app detected the pad after searching then automatically told me it needed updating.
After update I had to re-sync but haven't had to since.

Yep, the impulse triggers do not seem to work with BluetoothI've just got the battery and cable kit and have noticed that it vibrates better with the cable attached and has proper accelerating (right trigger) and braking (left trigger) vibrations now and i'm sure it didn't do that via Bluetooth.
Whats yours like? I was playing Forza Horizon 3.
What distance are you guys getting on Bluetooth?

I think I'll give it a try with the cable and see what I'm missing.
Not very far!
I'm getting about 2-3 metres but luckily that's enough to use with my PC
My Xbox one s pad vibrates and I'm on Bluetooth.
When playing Forza I can feel them as expected
