Ah, watching it on iPlayer so a bit behind, they've just confirmed it.

I have no idea how you could a) stand so far ahead of your rivals and not realise and b) stand just before the changeover line and not realise. That's unforgiveable for any relay athlete to do, regardless of the level.
 
I missed it, so will have to rewind to spot it, but as you say usually it's fairly obvious, although the italians and brazillians only just got back into position for their changeovers.
 
Just watching the replay of the women's 100m heats and the track looks ridiculously quick for the shorter events. I thought DAS might have a chance of a medal but Elaine-Thompson, Fraser-Pryce and Ta Lou all jogged to times around 10.8.
