It's very deliberate, the guy who designed the aliens intended for the xenomorphs' heads to resemble the human penis for example. There's a lot of phallic and sexual influences over the design of the aliens.

HR Giger is his name - an awful lot of his art involves sexual organs depicted in not entirely obvious ways. Studied him for A level art 10 years ago the guy is a genius imo- men being terrified of flying vaginas which in turn impregnate them with a living organism ultimatly leading to their deaths....
He didnt have a great relationship with his mother iirc
A QTE is essentially an interactive cutscene. You have no actual control over what is going on, but at certain points you have to press a given button to progress. Doesnt sound ideal, but as you say if its done well then hopefully it wont be a game killer
